Harden spoke to the Illinois Risk Management Services Annual Meeting on October 2 about hybrid (electronic and paper) medical records and how to avoid delays, inefficiencies and possible court sanctions in responding to requests for a hospital chart. The audience was hospital risk managers from institutions who are members of Illinois Provider Trust. The proliferation of electronic devices to create or modify electronic records is a challenge for medical records departments. Illinois law defines a “document” as anything in computer storage, and this means that the electronic system must capture all modifications doctors and nurses make to the chart.
